Using the issue/pull-request template #275

I think that GitHub's issue template and pull-request template feature guide users how to write and report the issue.

Here is the other project's example.

I have seen the maintainers countless comments such as "Write the application's version", "Show the commands to reproduce", "Show the actual result", and etc in some projects. I think creating the template improves the situation.

How do you think?

@junaruga -- It's a good idea, but unfortunately I do not have the required permissions to add it. I'll have that looked into.
