Delete functionality - in draft mode #730

The delete option is provided only in draft mode.

Currently the delete does not work if the item has dependencies - for e.g. if a 
question has attachments or versions.

The delete functionality needs to be rewritten to become a "cascade" 
delete.i.e. it must delete versions, attachments and other dependencies before 
deleting the item.

Please note that it should also work when there are cosignatories involved.
