Remove the add option from the object menu, adding is done in the edit panel. The add button doesn't work completely in combination with the modal edit dialog (the added item doesn't open for edit, since it only exists after the add and the command is fired before the add).
Adding in the edit panel is better, because it gives more information about the context.
The add button will remain for situations where it is needed (like the response flow).
