AS A senior citizen

I WANT TO create input fields as a carousel on the search/main page for the following fields

SO THAT...

the user to retrieve information for their locality on physicians.

When all the fields have been created in order for the user to locate a local physician:

• Zip code
• type of provider (e.g. general physician, specialist, etc.)
• Physician gender
• Covid risk level (low, moderate, severe)... these will definition and business logic.
