Create an online form that generates a template for the lab #21

Create a form for data provider intake.

This form should :

  1. Ask a few quick questions about site location, default sampling, measurements that are taken,
  2. Then it should save this tombstone information directly to the DB, or other ()
  3. Output an excel file with a single sheet that is a template for that lab
  4. tell them we will be in touch to arrange file drop

@DougManuel can you provide your thoughts on this as a way forward?

We've started Survey Monkey forms that could be used for this purpose. We had envisioned initial lab, site and sample data that is then used to prepopulate an input form for sample measurement.

I've started a new "Template and input forms" MD. I'll put links to those draft forms.

PHAC has started a MS PowerApp. There has been discussion in Ontario using ArcGIS survey123 and a draft has been generated.

However, at this time the focus is generating Excel templates because Excel is the most used method of supporting assays (calibration, ct plots, etc.)

Is this now something that would be reasonable to add to V2.0? Or should we push this to a later version?
