Baseline risk #54

Another important piece of the model is the baseline risk of the event at time t. I have modified the 'fine_and_grey_model_example.Rmd' file to show how this is obtained. Yulric has asked for value to be added as a row to the reference file, labelled as, for example, 'H0_5YR' for DemPoRT, as we are using 5 year risk

I also made some other minor changes to the fine and grey model example, including using the rcs2 function to names the variables created by the rcs function
