Need better error reporting for file access problems #112

Particularly with the 1.1.3+ updates to the phone, Books needs to better
handle and report permission denied and other such file access errors. 
Crashing isn't the most helpful of diagnostic messages...

Failed delete is reported as of r382.  Failure of file open still needs to be 
detected.

Fail on open is reported as for r420 or there abouts.
