Redirects tbi severity as a scalar to represent glasgow coma scale

so i don't see anywhere where gcs is set on the patient, you could just copy and paste that howto code into nervous.cpp and set it as a function of that as part of this pull request, seems like this is just to make that data avalable? currently it will just always return 0 per these commits

I would refactor this, If you can't set it no need to have a Has function at all, just calculate the value when they call Get and don't even keep a member or change the serialization routine.
