Turned the index.html into a header.php, index.php, and footer.php.
Turned 404.html into error.php which handles more cases and obfuscates
403 errors as 404 errors. Also updated favicon.ico and added a private
folder that is not publicly readable. Mostly this is a test commit
since I don't have much experience with github.
