Emergency referral - Yes/No

Reason for referral

• Pregnant woman with pre-existing medical condition

• Preeclampsia or Eclampsia with other complications

• Placenta praevia

• Abruptio placenta.

• Diagnostic tests (specify)

when the option pregnant woman with pre-existing medical condition is selected, that's when the other options should show. The following below are the options that should be shown. Options should be a checklist ◊ Asthma ◊ Cardiac condition ◊ Diabetes mellitus ◊ Epilepsy ◊ Hypertension ◊ Hyperthyroidism ◊ Moderate to severe anaemia (< 8g/dl) ◊ Previous or current coagulopathy (eg. DVT) ◊ Renal disease ◊ Sickle cell disease ◊ RH negative

When this option Preeclampsia or Eclampsia with other complications is selected, the following options should be should shown. ◊ Abruptio Placenta ◊ Coagulopathy ◊ HELLP syndrome ◊ Renal failure, ◊ Thrombocytopenia ◊ Unconscious patient

When Diagnostic tests (specify) has been selected a text field should be shown to allow the user enter the type of test that has to be conducted.
