The field should take in month and year only. Field label should be Year of Delivery/Termination

Infact, because there was a requirement to add a field named "Estimated Year of Delivery" we can leave this field as is just name it " "Date of Delivery/Termination" if user does not remember the date that is when they can fill in the field "Estimated Date of Delivery". The last field should only appear if a some check box (can be named "Cannot remember") is checked. The two fields can not appear at the same time.
