Realign the Previous Pregnancies section to have the following fields and have it aligned to ANC Card.

Fields

• [ ] Date of Delivery/Termination
• [ ] Add a field labeled "Estimated Date of Delivery/Termination if client does not remember the date. (there should be a field or checkbox that activates this field when checked) the filed should be "Date unknown" (see date of birth field)
• [ ] Duration (months) - should only allow 1 to 10
• [ ] Mode of delivery - to take in the following options; "Normal Vertex Delivery", "Assisted Vaginal Delivery", "C-Section". (Validation: Duration in the above field should be 7-10 for the field to be active or show)
• [ ] Type of Labour - options to include; "Induced" or "Spontaneous" (Validation: field should only be active or show if Duration provided is 7-10, and a delivery mode should be provided)
• [ ] Sex of Infant - options should be "Male" or "Female" (Validation: field should only be active or show if Duration provided is 7-10, and a delivery mode and type of labour should be provided)
• [ ] Birth weight - should be in grams. (Validation: field should only be active or show if Duration provided is 7-10, and a delivery mode and type of labour should be provided). Weight can only be between 800 and 6000 grams.
• [ ] Outcome - options should be "Live Birth", "Stillbirth", "Abortion/Miscarriage". Validations/Constraints: If Duration provided is 7-10, the option "Abortion/Miscarriage" should not be selectable. If Duration is 1-6, the options "Live birth" and "still birth" should not be selectable.
• [ ] Remove the field "Puerperium" completely

The label Duration that is displayed as field placeholder should change to "Duration (months)" or add "months" in brackets to the label Gestational Age.

And why is the word "Age" starting with a small letter 'a' in the pregnancy 1 form and starting with capital 'A' on the pregnancy 2? Small detail but uniformity is important.
