The case closure form should only have two options, namely; “Graduated (Household has met the graduation benchmarks in ALL domains)” and “Exited without graduation”. Selecting the option “Exited without graduation” should trigger a field labelled “Reason for exit” with options “Transferred to other OVC program”, “Lost to follow-up”, “Aging without transition plan”, “Died”, “Moved (Relocated)”, “Other”. Choosing the Option “Other (specify)” should trigger a text field that will require the user to enter the other reason. #1114
