The field “Do you have the ECAP ll household ID ?” should come above the field “ Date enrolled in USAID ECAP ll Program ”, so what this means is that when we get to the section PMTCT Mothers Details we need to first ask the question “Do you have the ECAP ll household ID ?” which will have the options “Yes or No”, if the option “Yes” is selected then display the fields “Date enrolled in USAID ECAP ll Program” and “ECAP ll comprehensive ID” , the name, client’s nickname which should be optional, age, date initiated on ART, Mothers SMH No, Address, Nearest Landmark and Phone Number but if the option “No ” is selected under “Do you have the ECAP ll household ID ?” then do not show “Date enrolled in USAID ECAP ll Program” and “ECAP ll comprehensive ID” just display the name, client’s nickname which should be optional, age, date initiated on ART, Mothers SMH No, Address, Nearest Landmark and Phone Number
