Graduation under case closure #1153

  1. At case closure, selecting graduated should ask for the RCMS's signature
  2. graduation on case closure should only be selectable if the graduation benchmarks assessments were completed
  3. Currently, when HH is closed the graduation cap Icon changes to a red house. Let the graduation icon persist even after case closure provided the reason given at case closure is graduated. This will be for easy differentiation of graduated HH's from exited without graduation HH's.
