switch to andre-dietrich/parser-combinators which has been upgraded to 0.19
tests modified slightly to remove precending "+" which elm doesn't seem to support any more
no parser modifications for 0.19, just changes to compile and pass existing tests

Is the example working for you? it's just blank for me when I load it

Yes example/index.html works for me in Firefox and Chrome
