exercise-like headings #78

The headings of exercise-like environments are not being styled. For example, see the bottom of

http://sl2x.aimath.org/book/aafmt/section5-4.html

Seems hard to believe that those were never styled, but then what made the styling go away?

Looks like the styling is appropriate in the exercise section, but not for exercises outside of such a section.

I think I can do it purely as a CSS change, but I asked Alex's ORCCA group for feedback because that is where I noticed it.
