Move AJAX Trigger off XSLT #1835

This is a step towards many goals but primarily I am aiming to remove the "registration scripts" from the XSLT, in fact any script loading at all beyond the main entry point.

 Having only one entry point makes bundling a lot easier.

Also fixed some style issues and a bug introduced in the previous PR.
