Neural data reading reorg #67

Removes TDT/HTK managers and moves their functionality in to HTK/TDT readers or neural/mark originators. Still need to update stim, but there are other problems in the stim pipeline that need to be worked out in tandem.

Makes a few bug fixes that help the tests get a little further along and adds a PR template.
