Originate failed error when placing calls

What steps will reproduce the problem?
1. I dial a number
2.
3.

What is the expected output? What do you see instead?

Instead of placing the call, I see this output in the debug info window:

Response: Error
ActionID: outcall_dial_action
Message: Originate failed

What version of the product are you using? On what operating system?

I've tried 1.55 and 1.60. Same behaviour

Please provide any additional information below.

I can connect, but I cannot place calls.

I have modified the manager.conf file to add the "originate" option but nothing 
changes, I've tried to use version 1.55 and 1.60 but it didn't solve the 
problems.

I've tried other solutions that I've read in the issues list like unchecking 
the "connect automatically..." and to put the extension password in the 
registry without success.

Is this still an issue for you?  If so, respond and let me know and I'll send 
you my manager.conf to try and help out.  Also, I have made some modifications 
to version 1.6 that allow you to pipe the callerID, name, time of call, and 
extension called to a command line or out to a URL if you're interested.  Also 
the caller id was not coming through at all which turned out to be a symptom of 
using newer versions of Asterisk (1.8) also resolved, but probably broke the 
ability for it to work properly on older versions

Hi, Did you get this resolved? I am having the same problem using outcall 1.6. 
Outcall connects but unable to place calls with originate failed error. As you 
did I have tried editing the registry to add the extension password but still 
no joy.
