What version of the product are you using?
1a5?  (Windows reports 1a5, application load screen reports 1a4, change date is 
the 15th...)See the end of this message for details on invoking 
just-in-time (JIT) debugging instead of this dialog box.

To enable just-in-time (JIT) debugging, the .config file for this
application or computer (machine.config) must have the
jitDebugging value set in the system.windows.forms section.
The application must also be compiled with debugging
enabled.

For example:

When JIT debugging is enabled, any unhandled exception
will be sent to the JIT debugger registered on the computer
rather than be handled by this dialog box.

Oh, and the application loads but the controller is still not showing up.

Uninstall Pad Tie and reinstall 0.1b1. You shouldn't have this problem again. 
This bug is actually a duplicate of issue #13.
