Implement custom javascript for surveys #370

As we can do with custom CSS, add custom javascript for surveys.

- The javascript file can be uploaded and must be non empty or it will be 
ignored.
- Once uploaded, the javascript file can be edited with a simple UI.
- The javascript will be added at the end of foundation.jsp so it can take 
precedence over all other javascript.
- The javascript file will be handled properly by the survey importer/exporter.
