Get Echo notifications working on Refreshed skin (and others later on) #84

Just putting this down for now as something to be done eventually. We should have Echo notifications up on Refreshed, and other skins as necessary, sometime next year. On Refreshed, they can be next to the username in the header. In Vector-based skins (Vector, Lia, Deepsea), they can be in the standard location, as seen on Wikipedia.

I've gotten this working in all skins globally now.
