Create file tests for conflicting options #26

With only one clean.md file, testing both balanced and unbalanced atx headers in live files is impossible. This isn't unique to balancing headers. Header style preferences conflict, as well.

For instance, clean_atx_balanced.md, clean_atx_unbalanced.md, and clean_setext.md files would need to be added. Just to name a few.
