QTMA to MIDI Conversion #12

These changes update sound.py to handle general QTMA to MIDI conversion with support for all core QTMA commands. An included example script shows how midiutil may be used to create MIDI files from the QTMA command list.

Happy to help :). The ability to extract data from the Delver archives has been really useful! I've been able to take these files and work on an updated version of the Cythera soundtrack which I'm planning to put up on cytheraguides tomorrow, so thank you for making this possible!
