Form not refreshing- Title & Groups #11

I updated the group/ title for a form and updated the plugin file and removed and re-installed the signup form widget to the sidebar but the group is still showing incorrectly. How can I repair this?

I'm sorry - I'm not sure why I wasn't emailed about this. Can you give me the URL of the form + the URL of the form as hosted by Salsa?
