Fields not populating email sign up form #43

Hello,

The fields in our email sign up form widget are not populating on our site. See under the text that says "Join OJPC!"

Any suggestions? screen-capture

Taking a peak at the source code I DO see the form - just no fields:

So my guess is this is a caching issue. The Salsa form seems very straight forward, so my guess is that WordPress is storing the old version of the form, before you added the fields.
