part 1: remove header cell, small fixes #103

Similar to #101, closes #102

No (or at least not many structural changes) - but did some edits to the text and code. I replaced tight_layout with constrained_layout.

There are still remaining header lines in Ex 2.1, 2.2, 4.0, 4.1, 4.2.. Should we open another pull request for removing those?

Ex 2.7 still introduces tight_layout, so if constrained_layout is better, we should be change that.

I just saw that this is done in the other pull requests. Only Ex 2.2 is missing. I can open a pull request for this.
