will start on this tomorrow. this is the one of the places to book a vaccine outside of pharmacy distribution and the MA pre-reg system.

waiting on #206 to land before working on this. maimmunizations.org is also a prepmod based system so I'll be able to re-use a lot of the normalizer code from the wa/prepmod normalizer.
