Change site title to include FindTreatment.gov instead of TreatmentFinder
Change site title on homepage to include Home -> Home | FindTreatment.gov
Change logo text to FindTreatment.gov
Update Google feedback form links and place in ENV var for easy updating
Update all references to TreatmentFinder to FindTreatment.gov
Update all references of findtreatmentbeta.18f.gov to findtreatment.gov

Hey @davemcorwin - I still see TreatmentFinder in the text logo. This may be still on your list but in case you missed that, wanted to mention it. Thanks!
