Add newborn screening codes from CPHI

Summary

Adds the newborn screening codes provided by the Center for Public Health Innovation. For now (and until we complete the work to allow users to add their own codes), these codes are added as a 5th migration on top of the existing migrations. Until then, some SQL inserts 🙈
