The current form layout presents some usability/flow issues, and the page overall needs some content edits as well.

Form

[ ] Look into better groupings for inputs (ex: primary and secondary contact are kind of jammed together and read L->R a bit strangely)
[ ] Confirm all fields are present
[ ] Confirm all options are present
[ ] Test submit the form from multiple devices
[ ] Test form validation and review error content

Moving to icebox. Not moving forward with Dynamics but these issues may apply to whatever we work on next.
