Delete the deprecated hl7-use extension

User Story

As a report stream dev, I would like to delete the hl7-use FHIR extension

Description/Use Case

There are cases in the OML/ORM mappings as well as the old deprecated mappings that use this extension. Rather than trying to fix all of those it would far easier to delete the old mappings and fix the mappings for ORM.

Dev Notes

There will be a separate ticket to delete the old mapping files. This ticket is just to update our long-term mappings to not use the soon-to-be-deleted mappings.

Acceptance Criteria

• [ ] The ORM FHIR-> HL7 mappings are fully updated to use the new mappings
• [ ] The hl7-use extension is deleted
