Add "How did you hear about us?" to our Healthcare Org Interest Form

Problem statement

As a ReportStream team member, I want to know how our senders find out about ReportStream so that I can analyze what communications channels are the most effective, informing our future service design strategies.

What you need to know

At the foot of the sender intake form, we will add a question similar to "How did you hear about us?" with a dropdown of common answers (ex. Word of mouth, CDC website, STLT) or write in their own.

Acceptance criteria

• [ ] Survey question content developed by Design
• [ ] Survey question reviewed with Account, Product, Tech Lead, and Support
• [ ] Survey question content finalized and handed off to Support

• [ ] Provide heads up to Support team about upcoming addition
• [ ] Draft of question & dropdown responses, review with Product/Account
