Hello James,

I have attached the original ELR message with the updates for OBX-17.1, SPM-8, SPM-4, PID-15 and below ARE questions regarding the OBX-5 and the pregnancy question.

I am little unsure and need to discuss to ensure I understand what is needed. Thank you for your support in the below questions 😊

Errors from 9/13 Adjust test message corrections/errors from Joel (ATTACHED) OBX-17.1: Needs to line up with a Testkit Name ID (Column M) from the LIVD table (https://www.cdc.gov/csels/dls/sars-cov-2-livd-codes.html) In ELR, it is "07613336100097", in POC it is blank Once you set OBX-17.1, make sure that OBX-3 corresponds to the test performed LOINC code in column F for that row in the spreadsheet. The code 181200003 for field SPM-8 is invalid. Values we're expecting are listed in the table under specimen_source_site_code here: https://github.com/CDCgov/prime-reportstream/blob/master/prime-router/docs/schema_documentation/hl7-hl7-ingest-covid-19-prod.md The code 309072003 for field SPM-4 is invalid. Values we're expecting are listed in the table under specimen_type here: https://github.com/CDCgov/prime-reportstream/blob/master/prime-router/docs/schema_documentation/hl7-hl7-ingest-covid-19-prod.md The code ENGLISH for field PID-15 is invalid. Please use one of the 3 letter codes (e.g. "eng") from https://phinvads.cdc.gov/vads/ViewValueSet.action?id=43D34BBC-617F-DD11-B38D-00188B398520 The code N for field OBX-5 is invalid. Our current release expects the test result to appear in Set ID = 1 of the OBX's. Are you able to reorder your OBX segments so the result appears here? We are working towards changing this, but do not yet have an ETA. CURRENT DIRECTION – PROV EVALUATING, BUT MOST LIKELY SOLUTION WILL DEPEND ON NEW CDC ALLOWANCE FOR THIS. OUR ABILITY TO CHANGE THIS/ SHIFT IS VERY LIMITED. For the pregnancy status AOE, 82810-3, please use the values 77386006 (Pregnant) 77386006 (Not Pregnant) 261665006 (Unknown)

If you could take a look at the attached message and send back to me highlighted in yellow what you are expecting here from the attached message. Highlight the result and then Highlight what field in the OBX segment you want it to appear, would be very helpful

The Pregnancy -

For this I am assuming the answer to the “Is patient pregnant” is, N^No^HL70136 and instead of N you want 77386006 is this correct?

So it would appear like this: 77386006^No^HL70136

Answers to questions in red below:

1.) OBX-5 not sure what is needed here…you want the result to be in OBX-1? If you could take a look at the attached message and send back to me highlighted in yellow what you are expecting here from the attached message. Highlight the result and then Highlight what field in the OBX segment you want it to appear, would be very helpful - This was a limitation of our system where the first OBX in a message needed to contain the actual covid result and not one of the AOEs. We did some engineering and this is no longer necessary, so you can ignore this comment.

2.) For this I am assuming the answer to the “Is patient pregnant” is, N^No^HL70136 and instead of N you want 77386006 is this correct? So it would appear like this: 77386006^No^HL70136 - Correct, we want to use the coded Snomed values for the OBX-5.1

A couple more things from this newest file:

1.) There were two OBXs that didn't have an OBX-3.1 value

2.) SPM-17.2 Could we remove this, it is causing issues on my end. The SPM-17.1 is all we need.

On your comment below regarding OBX-18 did you mean OBX-17 because there was nothing in OBX-18……

Hi Lottie,

You are correct in that OBX-18.1 was blank in the message from you. What we are looking for is that it be populated as described below. It looks like in your original message this information was there in OBX-17 in some form, so we probably just have to make some slight modifications to get it in the format we are looking for.

Is it possible to layout for me what column values you want in each OBX-17 or OBX-18 Fields from the LIVD-SARS-COV spreadsheet.?

For Example

Update (from James) 2 weeks testing with ReportStream testing env OBX 17 & OBX 18 fields have not been matching/ TODO James matching test “livid” table records (working with Lottie) – looked at MAC export from Miranda during our call –James needs to match info to test kit for the “livid” table – is there a way to match the test codes to the LONIC code output – should be going through the interface – DONE : Miranda will do an export of test codes and LONIC codes that are associated with them – then James can do the matching and work with Lottie to map the interface (there will be 32 entries). Language field not behaving as expected SE BELOW: Report Stream/James investigating still working on Lottie Question: Working with James on SPM segment (specimen type) not matching. Get “b” and “c” resolved and we should be done with testing. LIVID TABLE NOT MATCHING ITEMS i. 2/13: Still matching up livid table items (Prov is providing device information NOT test kit items = impossible to match the Livid table)

                                                         ii.      James inquiring with CDC to NOT requiring Livid table matching. (Stringent matching being loosened post covid; Loinc codes not necessarily used the same way).

                                                       iii.      We confirmed our Loinc code submissions are currently being accepted by all 9 of our states.

                                                       iv.      BEST WAY TO MOVE FORWARD: TODO James investigating how to de-couple stringent Livid matching.

                                                         v.      Option 2 – see if we can get OBX18 value to match (with equipment UID – colmum “O”) - -> may be able to find unique values in the LIVID table.

                                                       vi.      Option 3 – Using the Miranda Xpert Xpress SARS-CoV-2 - Fact Sheet for Patients (fda.gov)

                                                      vii.      IF WE CAN’T FIND A WAY THROUGH THIS QUICKLY, James is telling us the priority is shifting at report stream, away from Covid only to all conditions.

Testing Lottie Gilmore: James seeing some of the current filtered items that were non covid?? TODO Lottie investigating. Also 204 status code being reported as an error.

Kevin needs vendor information for Vendor Assessment – 2/6: Still in progress – info sent to ReportStream 2/2 Oza, Riddhish P riddhish.oza@providence.org sent Stephen, tfo3@cdc.gov the 35 page Vendor Assessment doc to fill out DUE 3/2/23

Working on crosswalk from Providence OBX-17 values to LIVD table value for OBX-17 and OBX-18. Also need to investigate issue with valueset for language.

Providence is no longer going to onboard to the COVID pipeline but is interested in onboarding to full elr when available. Check in set for 3 weeks from now.
