Remove manage ordering provider capability from facility level

Description

Follow-on to managing ordering provider at org level and selecting ordering provider on test card

Acceptance criteria

• [ ] Ordering provider fields are removed from manage facility pages
• [ ] Update backend so ordering provider can't be added to facilities
• [ ] Messaging to communicate that ordering provider has moved (needs content work, work with Laura Kerry)
• [ ] this feature will be behind a feature flag - multiple-ordering-providers

Design considerations

• Preceding tickets will be hidden behind a feature flag. Once this ticket is done all of those features will be turned on (at this point, users will have ability to select ordering provider on test card)
• At this point, the user will only be able to manage ordering providers at the organization level, so we need some copy on the facility page telling them that this feature has moved
• There will also be duplicates (in test card and on ordering provider page) because this comes before the "consolidate existing providers" tickets. Not sure if we need copy to address this
• Do we want to communicate about this additional field elsewhere? (on test card?) to alert user to changes to test card.

For the question about communicating this change elsewhere, I feel like I don't have enough sense of how we expect users to interact with this feature yet, but at the very least we can create a "What's new" card — will drop some copy in the Google doc and tag you.

Ordering providers now in organization settings To make it easier to manage, SimpleReport has moved this feature to the Manage organization tab. Find all existing ordering providers and add new ones there.

New tab:

Remove after consolidation of duplicates

Ordering providers moved from facilities Ordering providers from all facilities have moved here. SimpleReport will combine any duplicates [timeframe — soon? in the next couple weeks?], or you can delete them. This won’t affect your existing reports.
