Allow users to designate unknown fields

Description

Many people getting tested for HIV are unhoused and may not have an address / phone number. In this case, users should be able to designate that these fields are unknown.

Notes:

• This is just for the single entry workflow, bulk upload will be in a separate ticket
• Miscellaneous note field will be in a separate ticket.

Acceptance criteria

• Add headers (H4) for phone, email, and address sections of the contact info (per figma design)
• For address and phone number fields, user can check a box to indicate that address and/or phone is unknown. When they do so, fields in that section disappear (and will reappear if box is unchecked)(including phone/text preferences)
• Follow RS HL7 guidance to complete these fields - https://docs.google.com/spreadsheets/d/1sSpDeKQdackzRS5IT58WmUWn1eq6pA1Rm57u0C5QpgM/edit?usp=sharing
• For street: populate with " Unknown / Not Given
• For city, state, zip code, phone: populate with ordering facility information, generate county from that information (how we're handling it today)
