Adjust layout and language for Forgot email?

• make the Username field shorter (using the standard short/half width)
• left align the Username field and buttons with the rest of the text on the page (shift to the right)
• make second word in heading lowercase "Forgot email?"
• revise message below heading: "If you have an existing DMPTool account, please enter your username and we will provide the associated email address."
• revise message below buttons: "If you have forgotten the username and email for your existing DMPTool account, please Contact us." (make Contact us a hyperlink)
• revise text in blue bar message at top of page as follows: We do not recognize the username ___. Please try again or contact us if you have forgotten the username and email for your existing DMPTool account."
• make this message yellow with an "x" icon (or whatever color we use for error messages since it represents a failed attempt)

just add the word "Error" to the error message for unrecognized user name (screenshot below). "Error: We do not recognize the username ___. Please try again or contact us if you have forgotten the username and email for your existing DMPTool account."
