DEATH constraint for records without death date #8

Some data partners have mortality data that does not include a date of death.

These records violate the DEATH table composite primary key and constraint for PATID + DEATH_DATE + DEATH_SOURCE. These records also violate the DEATH_DATE (required, not null) constraint.
