Elsevier, JATS: important fixes & clean-up #41

Changes JATS affiliation parsing (fix for comma separation).
Fixes updating Elsevier records with new PDF/A file.
Fixes sorting packages from Elsevier with name "CERNR".
Refactores package extraction code.
