Medaka VCF patch fix #31

This PR fixes parsing of Medaka VCF with low/no coverage variants, and also updates the HISTORY.md, README.md and setup.py.

version bump to 0.1.1

@nhhaidee this PR has a fairly minor change to the code to fix parsing of Medaka VCF files which can have 0 depth variants called (not sure why that happens). Let me know if things look good to you.
