Adventure inspiration not producing any artifact

Some areas don't provide any artifacts when visited by adventurers. I'd need to know which exactly and if it's occasional or systematic.

I suspect it's mainly new and wastelands areas. I'll need testers to send out as many adventurers as possible (preferably with high skill and looking for unique stuff), and report which ones aren't working.

I'll be using this post to mention all the adventurer inspirations that result in the adventurers returning with nothing when sent with the intent to discover something unique. (will keep editing it when i discover more)

Observations:

1: For all the regions mentioned below it seems that the pool for unique artifacts is empty and that is why the adventurers say that they successfully found an artifact and yet return empty handed

2: It's really seeming like it's all the areas added post v3

3: During all these empty inspirations the characters (upon clicking on their "Go to location" interaction) appear to be in the bottom left corner of the map and seem to be located in nowhere as can be seen in the screenshot below where it says "Located in blank"

This is also my guess, but it's very perplexing because all these regions have relics defined, if not unique artifacts

Here is a region I had, I missed the original location that was mentioned but they changed direction and I allowed it.
