Error when importing question_item/response fragments #475

I have been able to load some question items (with response domains/code lists attached) for many questions, but for many I receieve the error 500 Internal Server Error.

I've attached two examples of one which works (several times) 132797 and one that is problematic 132807.

It will not load 132807 when I try in the live https://archivist.closer.ac.uk/instruments/mcs_07_cpa_new/build/questions and I have tired to replicate this on the staging, but I only receive the error the second time instead of it creating a _dup? https://archivist.closer.ac.uk/instruments/mcs_07_cpa_new/build/questions
