Update error messages for CSV formats #18

Error messages include additional explanation describing how to fix the error. Specific error messages are shown for violated conditional checks.

Some of the error messages are not updated. These messages were not in the most recent set of changes to error messages.

Looks good - lots of error message refactoring.

General comment: We are removing errors identifying the specific issue with an NDC is encoded and the lack of either the drug unit or drug type being omitted. This has been rolled up and combined into a single error.
