Cleanup of notebooks and stray files #8

Changes included in this PR:

Remove commented-out code from notebooks
Remove calls to !ls that will fail when the notebook is run remotely.
Add code to echo what files each notebook is writing.
Remove env.py, as it is no longer used.
Add a .gitignore to the data directory to ensure that JHU data doesn't get checked into our repository.

Edit: Additional changes pushed

Move static input files for notebooks to notebooks/inputs
Move location for writing output files to notebooks/outputs
Update descriptions of file locations at the top of notebooks.

Edit: More changes pused

Updated U.S. data pipeline to reflect new input and output file locations.
