This PR contains changes for the v1.2.0 release of the app, including:

Enhancements

Adds new bar chart showcasing # of youth by (ungrouped) exit destination; closes #94
Updates box titles to include the word "Group" where the data has been grouped to a level higher than what's represented in the data; 863706341b3a14e05e028aad05bb5b68b51704c0, a05d8cf9097c36ac7fa45ffe4867ead3ded74c54

Bug Fixes

Data is de-duplicated prior to populating the first stacked bar chart on the "Disabilities" page; closes #92
