[BUG] Multiple entries for a single subscriber id are possible #826

Ensure that an individual subscriber id can appear only once in the subscriber table

Server side fix deployed to DEV and TEST. The exact problem may be difficult to replicate in test, however it needs to be tested to at least ensure that the basic functionality is ok.
