I'm going to have to chime in and say I have the exact same model and have the same issue.
Tried burning the disk using both burning apps listed, at the slowest speeds my drive allowed (2x).
Also have been dinking around with my Laser on my Slim as it was having some issues playing PS2 games and DVD movies, but after adjusting the DVD Pot down (increased the OHM from 950 to 1100, now I can read my PS2 games that I couldn't earlier. However I still cannot play DVD Movies so far so more adjustment or maybe a new Laser is in order. I did adjust it down to 750 for a short time but that didn't seem to help and only made the drive seem to act a little crazy.
My two main games, GT3 A-Apec and GT4 were the main games that I got the PS2 for. A-Spec stopped being able to be read until I adjusted to 1100 today. GT4 has worked fine the entire time.
What I don't get, is this PS2 played both Games and Movies fine than sat unused for a extended period of time. Come back and even after cleaning it no longer reads disk correctly. Having the same problem with a barely used X360. Why do drives seem to die from old age? I thought it was supposed to only be the Disk?!?

I've tried all of the troubleshooting steps, nothing seems to work. I tried writing with PowerISO at 6x speed and CDBurnerXP at 1x speed with DVD High Compatibility. All of my DVDs are DVD-R
It should probably be noted that some DVDs work and some don't. For example, Regular Show: Fright Pack is recognized, but my Simpsons DVD is not.
