I see that you get the 404 error jquery.js?ver=1.12.4:4 POST https://realitymarketinggroup.com/clients/vycelogistics/cf-api/CF5b2ef4735e9eb/upload/ 404 (Not Found)
Does this continue after re-saving permalink settings?
I seem to still be having an issue. I have changed the permalink settings. Might there be another option to adjust this problem with attaching documents?
Looks like it works now! I just readjusted the permalinks to post name and it seems to be working.

I just had the exact same error message with attachments. After saving the permalinks (Settings > Permalinks), the error did not appear again and form submission works as expected.
New to Caldera Forms. How do you attach files to emaill?
