rename Malevo to ConfusionFlow #183

Do we want to rename everything?

Including:

[ ] Repository
[x] App name in header
[x] Caleydo App URL

As we'll move to a different repo in the future anyway we could keep malevo as repo name for now but the other stuff should definitely be changed.

Just realized we should really keep the repo name as malevo for now, as the whole yeoman setup, phovea stuff, etc... will have to be changed and it's not really worth the hassle.

Stuff will likely break.

I would suggest that too. Its very common in software projects to have working titles in repo names etc.
